Audrey has been a Registered Nurse for 24 years and began her nursing career at Peninsula Regional Medical Center in the ICU and Cardiac Catheterization Lab. She went on to work in Nursing Leadership and then, as a Hospice Consultant for Seasons Hospice and Palliative Care in Sussex County, DE. Most recently, she was the Transitional Care Coordinator at Nanticoke Memorial Hospital, ensuring patients received quality, coordinated care across the continuum.
She has dedicated herself to ensuring that patients and their families receive the information and knowledge they need to receive the best services they are entitled to. She prides herself on providing exceptional communication and customer service. By collaboration with the entire care management team, she ensures that each patient receives the patient-centered care they require.
A local to Salisbury, Maryland for 34 years, she has undergraduate degrees in Biology and Nursing from Salisbury University. Believing in the importance of being a lifelong learner, she has taken graduate Nursing Leadership courses at Wilmington University and is currently pursuing her certification in hospice and palliative care nursing.
In her free time, Audrey enjoys exercise, a daily meditation practice and yoga. She obtained her 200-hour Yoga Teacher Training in 2018 and enjoys sharing the benefits of this practice with others.
Hospital Liaison Delaware
Tisha started working for Peninsula Home Care in October, 2018 after working as a nurse for eleven years. She has 23 years of experience in the medical field with roles as a medical assistant, medical office manager and sales.
In her role as hospital liaison, Tisha serves as the connector between Peninsula Home Care at Nanticoke, the hospitals and patients. She works closely with patients, family members and caregivers through the hospital to home navigation process. Tisha offers support through her collaboration with care coordinators and clinical managers to customize and implement a plan of care that best fits the needs and goals of each individual patient.
Tisha graduated from Delaware Technical Community College with degrees for medical assistance and licensed practical nursing.
Tidal Health Liaison
Ashley Smith has been in healthcare for over 19 years with twelve of those years practicing as a nurse. She started with Peninsula Home Care in 2019 as a home health care nurse and in 2021 became PHC’s TidalHealth Liaison. Ashley spends most of her days at the hospital where she builds strong relationships with the staff and patients. In her free time, she enjoys spending time with her children on and off the baseball field.
