Establishing “A Plan of Care”

A plan of care outlines the services and treatment your doctor, home health nurse or physical therapist and you/your family decide you need. It will guide your recovery and include:

  • A list of needed services
  • The professionals needed to provide these services
  • How often you need the services
  • Equipment needed
  • Food/nutrition guidelines
  • Anticipated results based on pre-set timelines

Peninsula Home Care will keep in touch with your doctor to report your progress. Your Care Plan will be reviewed regularly (at least every two months) and updated based on your progress.


Obtain Services

If your doctor prescribes Home Health Care, you (or your doctor) can call one of our offices to order services and to verify Medicare coverage. (You can also call with Medicare directly at 1-800-MEDICARE if you prefer.) If your claim is not Medicare certified, our team will help you check with your personal insurance to determine if your services will be authorized and covered.
