Case Study - Jack Fisher

Jack Fisher

80 Years Young and Celebrating…Thanks to Peninsula Home Care

Jack Fisher, of Seaford, Delaware didn’t know if he would have the opportunity to celebrate his 80th birthday at home with friends and family. One month before the big day Jack was waking up in an in-patient rehabilitation facility after being hospitalized for congestive heart failure and kidney problems.

“I just wanted to go home,” said Fisher.

He made it there but knew he still needed help managing his condition at home.

“I didn’t want to have to go back to the hospital anytime soon.”

Fisher knew exactly what he needed.

“Peninsula Home Care,” said Jack. “It wouldn’t be the first time. Both my wife and I received exceptional care from Peninsula Home Care – once when I broke my ribs from a fall and then again when she had a knee replacement. Why would we go with anyone else?”

The most recent CHF episode required Jack to require nursing, physical therapy and home health aide services from Peninsula Home Care.

Crystal Fensick, a register nurse for Peninsula Home Care was assigned as Jack’s case manager. She said she couldn’t ask for a better patient.

“He did everything he was supposed to do,” said Fensick. “He was compliant with his medication. He bought a new scale and weighed himself every day. He even bought a blood pressure cuff. Mr. Fisher helped us help him.”

“If she wouldn’t have identified that fluid in my lungs and called my doctor, I may not be here today,” said Fisher. “Crystal was helpful, caring, professional and made me feel comfortable with the care I was receiving. She taught me what I could and could not do with the condition I was in. We referred to the CHF Zone Chart that showed me what signs and symptoms to look for. I stayed in the green with the exception of one time when I gained more than four pounds in one week. But Peninsula Home Care called my doctor and we took care of it immediately. I have been stable and feel great since my discharge.”

“He wanted his independence back,” added Fensick. “He wanted to stay at home and recover to a point where he could enjoy his family and his life. I am so glad we could be there to guide him to get to that point.”
